Visit Checklist

New Patients

  • Please arrive at the office at least 20 minutes prior to your new patient scheduled appointment and make sure to bring your driver's licence, insurance card(s) and new patient forms (filled out completely).

Existing Patients

  • Please arrive at the office 10 minutes prior to your scheduled appointment.

  • Be sure to bring:

    • ALL bottles of medications, vitamins and supplements (the actual bottles, not a written list) to EVERY office visit. This includes all prescription medications (especially those not prescribed by Dr. Karam); all over-the-counter medications (Advil®, Tylenol®, Imodium®, allergy, or cold medications, etc.); along with any vitamins or supplements you take.

    • Your driver's licence/state ID card and insurance card(s).

If you are unable to keep your scheduled appointment, please let the office know 24 hours in advance by phone: (847) 427-2100, or through the Patient Portal. By giving our office advance notice of your cancellation, we can offer your time slot to another patient. If we don't receive a cancellation notice 24 hours prior to your appointment, a $50.00 no show/late cancellation fee will be charged.

  • When you call our office for a refill, please be ready to provide us with needed information such as the name, address and phone number of your pharmacy, along with the name of medication requested, dosage, and directions.

  • Please call your pharmacy to see if your medication is ready to be picked up, not our office. Your pharmacy will have an easy-to-use, phone menu that will quickly allow you to check on the status of your prescription.

  • Copayments are collected before your visit. Please come with the payment ready and remember we do not accept any credit card charges under $25.00.

  • Our office is not able to collect payments through the Patient Portal.

Dr. Karam requests that you bring ALL bottles of medications, vitamins and supplements (the actual bottles, not a written list) to EVERY office visit. Having your medications with you at the time of your visit assures that our records of your current medications and dosages are up-to-date and allows us to check for any medication discrepancies, or if a new medication is prescribed that there will not be any interaction with your other medications.

    The HIPAA Privacy Rule provides federal protections for personal health information held by covered entities and gives patients an array of rights with respect to that information. At the same time, the Privacy Rule is balanced so that it permits the disclosure of personal health information needed for patient care and other important purposes.
